Symptom Diary


A Symptom Diary is one of the best ways to keep a detailed record of your symptoms. By taking a more active role in controlling your symptoms, you ensure effective communications with your doctor, nurse or pharmacist and selection of the best treatment option for you.

Information to include in your Symptom Diary

Keep a daily log and record:

  • When you felt nauseous or vomited.

    - How bad were these symptoms? Rank them from "Mild" to "Moderate" to "Severe"

    - How many hours or days after your chemotherapy or radiation treatment did these symptoms occur?

    - What medications did you take on the days you felt nauseous or vomited?

  • Any foods you ate that helped lessen your nausea or made your symptoms worse.

  • All other medications you took, including vitamin supplements, herbal supplements, aspirin, etc.

  • All prescription medications you took and how these medications helped your symptoms or made them worse.

  • Any activities that you think may have caused your nausea and vomiting. Try to avoid these activities.

  • Any activities that help take your mind off your symptoms. Try to increase these activities, which may include:

    - yoga

    - watching your favorite TV shows

    - talking on the phone

    - being out in the fresh air

    - playing video games (especially for kids)


Tips for Keeping a Successful Symptom Diary:

  • Make your Symptom Diary a daily part of your routine; always carry it with you and write down any change in symptoms or events.

  • Show your Symptom Diary to your healthcare professionals as soon as you start it. Inform them that you’d like to review this with them on each visit or check-up.

  • Remember, the more you information you record, the more useful the Symptom Diary will become as your move through your treatment course.
